Job Description:

We are seeking a highly skilled Renal Health Practitioner to join our team. As a Renal Health Practitioner, you will work collaboratively with interdisciplinary teams to deliver comprehensive renal health care, improving health outcomes and enhancing patient experiences.

  • Conduct thorough assessments and develop individualized care plans, prioritizing patient-centered care.
  • Collaborate with healthcare teams to coordinate care, ensuring seamless transitions and optimal health outcomes.
  • Provide education and support to patients and families, promoting self-management and empowerment.
Required Skills and Qualifications:

To be successful in this role, you will need:

  • A current Registered Nurse License in the state of practice.
  • At least 1 year of experience in dialysis or ICU, CCU, ER, or Med Surg nursing.
  • Basic Life Support (BLS) certification.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
