Job Title: Nurse Navigator

We are seeking a skilled Nurse Navigator to join our team at ChristianaCare in Newark, DE.

About the Role:

The Nurse Navigator will play a critical role in coordinating care for patients and their families, ensuring seamless transitions between healthcare services and promoting optimal patient outcomes.

Key Responsibilities:
  • Coordinate referrals from within and outside of Christiana Health Care, ensuring timely and effective care delivery.
  • Assess patient needs and match them with appropriate services, facilitating referrals and scheduling appointments as needed.
  • Triangulate family/caregiver phone calls, messages, and determine disposition of calls, coordinating response with Palliative clinicians based on clinical assessment.
  • Organize inpatient palliative census and daily workload, evaluating and triaging consults in collaboration with the hospital team.
  • Provide psychosocial support to patients, families, and caregivers, facilitating referrals and promoting self-advocacy.
  • Collaborate with the outpatient team to ensure seamless transitions and promote continuity of care.
  • Support discussions with patients and caregivers related to goals of care and end-of-life treatment decisions, facilitating scheduling of family/provider meetings.
  • Apply knowledge of insurance processes and their impact on care delivery, ensuring appropriate referrals and patient care decisions.
  • Monitor progress in achieving defined patient-specific, clinical outcomes through performance improvement activities and outcomes.
Requirements:
  • BSN required; Masters in nursing preferred.
  • RN license or eligible for licensure in the state of Delaware.
  • Three or more years of combined clinical experience in clinical and/or administrative experience in case management, clinical trials, older adult, palliative/hospice care preferred.
  • Nursing Cert - Certified Hospice and Palliative Nurse (CHPN) - Hospice & Palliative Credentialing Center within 3 years of hire.
