Job Summary:

We are seeking a skilled Nurse Advocate - Hospice Care to join our team at Traditions Health. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for providing compassionate care to patients with advanced illnesses.

Key Responsibilities:
  • Assess patient needs and develop individualized care plans
  • Provide emotional and spiritual support to patients and their families
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Document patient progress and communicate effectively with healthcare providers
Requirements:
  • Graduate of an accredited school of nursing
  • Valid license as a Registered Nurse (RN) in the state of employment
  • Minimum 1 year of experience in hospice or home health care
  • Excellent communication and interpersonal skills
