Position Overview

Key Responsibilities

1. Assess available resident information and determine its relevance to the evaluation and treatment process.

2. Perform and document a comprehensive functional assessment of residents, following standardized protocols. Evaluate home, vocational, and equipment requirements.

3. Create and execute a personalized treatment plan tailored to resident needs, incorporating realistic functional goals.

4. Adhere to evaluation, treatment, and documentation standards while maintaining accurate and timely departmental records.

5. Guide Assistants in treatment methods and provide close supervision during resident interactions.

6. Record a progress note in the resident's file after each treatment session.

7. Adjust treatment plans as necessary throughout the therapeutic process.

8. Summarize the treatment course in a clear and concise discharge report using the prescribed format.

9. Introduce residents to program services and treatment protocols suitable for their needs, while informing them of any potential risks involved.

10. Collaborate with nursing staff, residents, and families to optimize the use of residents' functional abilities.

Qualifications

1. Bachelor’s or Master’s degree in Physical Therapy.

2. Valid license in the state of practice.

3. Preferred experience in a setting serving a similar demographic of residents.

4. Relevant certifications and demonstrated expertise in specialized areas, if applicable.

5. Strong verbal and written communication skills, along with excellent interpersonal abilities.
