We are seeking a highly skilled and compassionate RN Clinical Manager/Patient Care Manager to join our team at Caretenders VNA, a part of LHC Group. As a RN Clinical Manager/Patient Care Manager, you will play a vital role in providing quality care to our patients and their families.
Responsibilities:- Supervise and coordinate clinical services to ensure high-quality patient care.
- Provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Review assessments and plans of care daily, per assigned workflow, and consult with clinicians with recommendations, as appropriate.
- Current RN licensure in state of practice.
- Current CPR certification required.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
