Job Title: Registered Nurse

Mass General Brigham is seeking a skilled Registered Nurse to join our Home Health team in Medford, Massachusetts.

Job Summary:

We are looking for a dedicated and compassionate Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health Registered Nurse, you will be responsible for delivering comprehensive nursing care to patients with various medical conditions, promoting independence, and improving their overall quality of life.

Responsibilities:
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Conduct thorough assessments, identify health risks, and provide education to patients and their families on disease management, medication adherence, and healthy lifestyle habits.
  • Collaborate with the interdisciplinary care team to ensure seamless coordination of care and effective communication with patients, families, and healthcare providers.
  • Administer medications, perform wound care, and provide other nursing interventions as needed.
  • Document patient progress, identify areas for improvement, and make recommendations for changes in care.
  • Participate in ongoing education and training to stay current with best practices, new technologies, and regulatory requirements.
Requirements:
  • Current Massachusetts license as a Registered Nurse.
  • Minimum of one year of recent nursing experience in an acute or emergency setting.
  • Home care experience preferred.
  • Ability to work independently and as part of a team, with strong communication and interpersonal skills.
  • Proficiency in electronic medical records and other software applications.
