This position involves direct patient care for three days a week, along with travel to designated areas.
Your responsibilities will include serving parts of the community while also managing supervisory and administrative tasks from home for two days a week.
The Lead Physician will guide a team of up to ten advanced practice providers, delivering home-based primary care to our senior clientele.
In addition to supporting your team, you will conduct in-home and telemedicine visits for high-acuity seniors three days a week.
This role emphasizes outpatient care, continuity of treatment, health maintenance, and disease prevention. You will maintain medical histories and records, refer patients to specialists as necessary, and advise leadership on significant strategic matters.
Utilizing independent judgment, you will navigate complex issues related to your duties with minimal supervision.
As a leader, you will provide direction and oversight to a team of advanced practice providers, ensuring effective collaboration agreements for nurse practitioners in the state.
Daily huddles will be part of your routine as you work within interdisciplinary teams, reviewing acute utilization events to minimize unnecessary interventions.
Your patient interactions will be focused on building strong relationships with seniors and their families through in-home visits and telehealth consultations.
Utilization of technology for EMR charting will be essential, along with ongoing management of prescription refills, home health referrals, lab follow-ups, and coordinated care.
Required Qualifications:
Graduate of an accredited MD or DO program.
Experience in leading a team of over ten advanced practice providers.
Proven track record in providing collaboration agreements for advanced practice providers.
8+ years of relevant experience and 2+ years in project leadership.
Must possess an active and unrestricted state license, DEA license, and board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
Current state driver's license and proof of auto insurance are required.
Experience in senior patient care is essential.
Active BLS certification through AHA or ARC is mandatory.
