This role encompasses a variety of responsibilities, including two 12-hour shifts weekly on the floor, Quality Assurance Performance Improvement (QAPI), and Infection Prevention (IP) tasks. This position includes a rotation for every third weekend and holiday, with an additional facility location premium of $2.50/hr.
Job Summary:
The Clinical Care Supervisor (CCS) is tasked with employing the nursing process—assessment, diagnosis, planning, implementation, and evaluation—to deliver personalized nursing care in a home setting. The CCS collaborates with a multidisciplinary team to plan, execute, and assess care. Responsibilities include documenting resident histories, symptoms, medications, and care provided in accordance with relevant standards and regulations. The CCS utilizes technology to enhance the quality of resident care, collaborates with team members, effectively delegates and supervises unlicensed personnel, and assumes overall nursing accountability for resident assignments. This role also involves leadership responsibilities on departmental and organizational task forces and committees.
The CCS demonstrates proficiency and practices within the full scope of nursing expertise, adhering to age and population-specific standards as designated in the care area. This position serves as the primary contact for QAPI measures and Infection Prevention initiatives. Effective communication skills are essential for collaboration, negotiation, and conflict resolution with peers, inter-professional colleagues, residents, and families. Strong clinical reasoning and critical thinking skills are required to analyze data and respond appropriately to changing situations, including coordination of care, resident assessments, education, triage, and various nursing interventions.
The CCS operates within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules of the state of practice and licensure. The organization recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses as published by the American Nurses Association as the foundation for nursing care delivery and professional conduct.
Qualifications:
- Bachelor's degree in nursing preferred.
- Graduate from a nationally accredited nursing program preferred.
- Minimum of two years of experience in long-term care or geriatric nursing preferred.
- Experience with the Minimum Data Set (MDS) preferred but not required.
- Must hold an unencumbered RN license with the State Board of Nursing or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
- Must obtain and maintain required department-specific competencies and certifications.
- Basic Life Support (BLS) certification required.
- Experience with the Resident Assessment Instrument (RAI) process preferred but not required.
