RN Case Manager

Job Summary:
The RN Case Manager is a key member of our interdisciplinary health team, responsible for coordinating patient care and ensuring optimal outcomes. This role requires strong communication and problem-solving skills, as well as the ability to work effectively with patients, families, and healthcare providers.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Coordinate with healthcare providers to ensure seamless transitions of care
- Monitor patient progress and adjust care plans as needed
- Collaborate with patients and families to educate them on their care and treatment options
- Ensure compliance with regulatory requirements and hospital policies
Requirements:
- Bachelor's degree in Nursing from an accredited school
- 3 years of experience in medical-surgical or critical care nursing
- 2 years of experience in care coordination or case management
- Current RN licensure in California
- Basic Life Support certification
- ACM or CCM certification preferred
Skills:
- Strong written and verbal communication skills
- Effective problem-solving and conflict resolution skills
- Ability to work independently and as part of a team
- Knowledge of Medicare and Medi-Cal guidelines and benefit resources
- Working knowledge of common diagnoses and procedures
Work Environment:
This role is based in the United States and requires a strong understanding of healthcare regulations and policies.
Pay Range:
$109.74 hourly-
