Job Summary

US Renal Care is seeking a highly skilled Dialysis Registered Nurse to join our team as a Home Hemodialysis RN. As a key member of our organization, you will be responsible for providing exceptional patient care and support to individuals undergoing home dialysis treatment.

Key Responsibilities
  • Provide patient training and ongoing support for home dialysis modalities, ensuring compliance with federal and state regulations.
  • Conduct home visits to assess patient environments and evaluate performance, making recommendations for improvement as needed.
  • Communicate patient assessment results, reassessments, and ongoing monitoring to physicians, team members, and other stakeholders.
  • Document all nursing services in the Electronic Medical Record, ensuring accurate reflection of patient status and interventions.
  • Participate in infection control monitoring, implementation, and recording, as well as emergency preparedness procedures.
  • Manage clinic and patient supply inventory, ensuring timely ordering to avoid fees and penalties.
  • Continuously seek to improve patient and quality outcomes through data tracking, critical thinking, and root cause analysis.
  • Administer medications as ordered by physicians and complete monthly review of patient medication profiles.
  • Perform on-call nursing services, nights and weekends, as needed or assigned.
Requirements
  • Knowledge of federal, state, and local laws and regulations, including healthcare professionals' practice act requirements.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Flexibility with staffing locations and hours to accommodate patient and US Renal Care home program needs.
Preferred Qualifications
  • Experience in home hemodialysis or peritoneal dialysis.
  • Knowledge of emergency equipment and procedures.
