We are seeking a highly skilled and compassionate Hospice Care Coordinator to join our team at Chapters Health System. In this role, you will play a vital part in delivering exceptional patient care and support to individuals and their families navigating the end-of-life journey.

Job Summary
The Hospice Care Coordinator will work closely with interdisciplinary teams to assess patient needs, develop individualized plans of care, and provide ongoing support to patients and their loved ones. This role requires strong communication skills, empathy, and a deep understanding of the complex needs of individuals approaching the end of life.
Responsibilities
- Patient Assessment: Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs to identify areas for intervention.
- Plan Development: Collaborate with healthcare professionals to develop individualized plans of care that address patients' unique needs and goals.
- Care Coordination: Work closely with patients, families, and healthcare providers to ensure seamless transitions between care settings.
- Education and Support: Provide education and support to patients and their loved ones on disease management, symptom control, and end-of-life care options.
- Communication: Maintain effective communication with patients, families, and healthcare teams to ensure timely and coordinated care.
Requirements
- Licensure: Current RN licensure in the state where employed.
- Experience: Minimum one year of experience in inpatient nursing or hospice care; internal candidates must have a minimum of six months of LPN experience in hospice/home health within an affiliate.
- Skills: Strong assessment and communication skills; ability to work effectively in a fast-paced environment.
- Certifications: Active BLS certification from the American Heart Association or Red Cross.
