Join Our Team as a Weekend Home Health Physical Therapist

BAYADA Home Health Care is seeking an experienced Physical Therapist to provide exceptional care to adult and geriatric patients in their homes and in Leominster, Boxborough, Harvard, and surrounding towns.

About the Role

As a Weekend Home Health Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to maintain their independence.

Responsibilities
  • Make visits to clients in their homes to assess their physical needs and develop individualized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
  • Develop and implement appropriate interventions, including manual therapeutic exercises, gait training, and balance assessments.
  • Continually assess and revise the Physical Therapy care plan and participate with nursing in the multidisciplinary care plan, as appropriate.
  • Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Qualifications
  • A current license as a Physical Therapist in Massachusetts.
  • A minimum of one year of recent clinical experience as a licensed Physical Therapist.
