About the Role: We are seeking a Licensed Practical Nurse to join our team at Vitra Health. This is a full-time position requiring 30+ hours of work per week.

Job Location: Boston Area, covering clients in Dorchester, Mattapan, South Boston, and Hyde Park.

Vitra Health is a growing company that values its team members. As a Licensed Practical Nurse, you will be responsible for conducting home health aide skills trainings, completing client assessments, and maintaining client charts.

Key Responsibilities:

  • Conduct home health aide skills trainings and competency evaluations
  • Complete initial client assessments, plan of care, and ongoing evaluations
  • Participate in care plan creations and reviews, as well as discharge plans
  • Assess and address motivational, psychosocial, and environmental concerns
  • Maintain and update client charts, medical records, and submit relevant information to authorized organizations

Requirements:

  • Valid Massachusetts LPN license
  • Minimum 1 year experience in an acute or home health care setting
  • Fluency in English and a second language preferred
  • Strong communication, writing, organizational, and time management skills
