**About the Role:**

We are seeking a highly skilled RN Case Manager to join our interdisciplinary team at Tufts Medicine Care at Home. As a key member of our team, you will be responsible for delivering quality hospice nursing care to patients and their families.

Job Overview:

The RN Case Manager serves as a vital link between patients, families, and healthcare providers, ensuring seamless coordination of care. Your expertise in palliative care and symptom management will enable you to provide individualized support to each patient, promoting comfort and dignity throughout their journey.

  • Coordinate care in a cost-efficient manner, prioritizing patient needs and preferences.
  • Collaborate effectively with other members of the IDT, including physicians, nurses, social workers, and chaplains.
  • Utilize the nursing process and current standards of nursing practice to provide high-quality care.
  • Provide effective patient and family teaching, empowering them to make informed decisions about their care.
  • Communicate regularly with physicians and Tufts Medicine Care at Home staff regarding patient status and changes in condition.

Responsibilities:

Some of your key responsibilities will include:

  1. Providing compassionate and personalized care to patients and families.
  2. Assessing patient needs and developing individualized care plans.
  3. Coordinating referrals to community resources and services.
  4. Managing complex cases, working collaboratively with the IDT to resolve challenges.
  5. Maintaining accurate and timely documentation, adhering to regulatory requirements and organizational policies.
