HIV Peer Navigator

The HIV Peer Navigator will play a vital role in helping patients engage and remain in care for HIV treatment. This position requires a strong understanding of HIV/AIDS, healthcare, education, outreach, and peer navigation. The ideal candidate will be a person living with HIV, with a passion for supporting others and making a positive impact in their community.
Key Responsibilities:- Provide one-on-one support and education to people living with HIV, addressing their unique needs and challenges.
- Communicate with patients via phone, text, or in-person, providing information on community-based services and resources.
- Work closely with the Rapid ART program director, Clark County Public Health, and DOH to ensure seamless care coordination.
- Assist clients in navigating mental health and substance use recovery systems, as well as overcoming social determinants of health and other barriers to care.
- Maintain awareness of cultural challenges and develop effective relationships with a diverse patient population.
- Accompany clients to community-based organizations and institutions as needed.
- Understand and follow HIPAA compliance policies and other rules of confidentiality.
- Monitor data reports and summaries of progress and status of assigned patients.
- Maintain professionalism and provide superior customer service at all times.
- High school diploma or GED required.
- 1 year of experience in HIV/AIDS, healthcare, education, outreach, or peer navigation preferred.
- Ability to work with diverse populations, including ethnic, racial, economic, and sexually diverse individuals.
- Strong communication and interpersonal skills, with the ability to work independently and as part of a team.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines.
- Knowledge of patient care processes, clinical procedures, and medical terminology.
- Knowledge about the lived experience of living with HIV and willingness to share that experience.
- Bilingual in English/Spanish or English/Russian.
