Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Optalis Healthcare. As an LPN, you will play a vital role in providing high-quality patient care in our Skilled Nursing Facility.

Key Responsibilities

• Evaluate assigned patients to assess their needs and develop individualized care plans
• Direct and supervise Certified Nursing Assistants (CNAs) in their daily care of residents
• Maintain patient comfort and safety by following safety, environmental, and infection control standards
• Represent the facility with integrity and adhere to our Core Values and mission
• Administer medications and other treatments as prescribed by physicians
• Assist physicians during examinations and procedures
• Promote patient independence by establishing and implementing care goals
• Resolve patient concerns and needs through multidisciplinary team strategies
• Stay up-to-date on nurse training and education, as well as facility policy and procedure

Requirements

• State-licensed Practical Nurse
• Valid identification
• COVID-19 vaccination or exemption
• Comfortable using a computer

Preferred Qualifications

• Prior experience in long-term care
• Prior experience with Infection Control
