About the Role

We are seeking a skilled Medical Social Worker to join our team at Carilion Clinic. As a Medical Social Worker, you will play a vital role in providing comprehensive care to patients and their families, addressing their emotional, social, and environmental needs.

Key Responsibilities
  • Conduct psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their health management, diagnosis, illness, treatment, and life situation.
  • Develop and implement comprehensive treatment plans utilizing social work interventions, coordinating with the care team and making recommendations for further treatment to achieve optimal patient outcomes.
  • Collaborate with other professionals to evaluate patients' medical or physical condition and assess patient needs, working with community agencies and resources to develop a comprehensive knowledge base of available resources for patients and families.
  • Provide patient and family education, support, and advocacy, making referrals for other services as needed or requested.
  • Participate in care transitions post-hospital and/or ED discharge follow-up to assess psychosocial and support needs and reinforce patient self-management expectations and goals.
Requirements and Qualifications

To be successful in this role, you will need:

  • A Bachelor's Degree in Social Work required
  • Experience in a healthcare setting preferred
  • Critical thinking and creative problem-solving skills to move patients effectively and efficiently through the continuum of care
