Role Summary:

The Community Support Services Consultant engages with participants in the Community First Services and Supports program, providing essential education on program models, regulations, procedures, and policies. Interactions may take place via phone or in the participant's residence. Consultants assist in crafting service delivery plans and conducting quality reviews, while also assessing service utilization against approved plans and offering recommendations for enhancing service quality.

Key Responsibilities:

  • Conduct home visits to a diverse range of participants, accommodating various home environments.
  • Dedicate a minimum of 70% of work hours to home visits.
  • Adapt to an increased home visit schedule as required by business demands.
  • Organize caseload efficiently to optimize travel time.
  • Travel 60 miles or more to participants' residences.
  • Maintain a satisfactory driving record and possess a reliable, insured vehicle.
  • Learn and effectively utilize PPL systems.

Essential Skills:

  • Independently manage a caseload with strong time management and organizational abilities to meet contractual deadlines.
  • Prioritize tasks in a dynamic, fast-paced setting.
  • Demonstrate excellent verbal and written communication skills.
  • Understand and advocate for the principles of self-direction.
  • Proficient in computer usage and Microsoft Office applications.
  • Collaborate effectively as part of a team, communicating respectfully and professionally with all members.
  • Ensure timely documentation, completing in-home visit notes during the visit and uploading them on the same day.
  • Project a positive image of Public Partnerships LLC to participants and stakeholders.

Required Resources:

  • Valid driver's license with a satisfactory driving record.
  • Reliable vehicle.
  • Secure internet connection.

Qualifications:

  • 1-3 years of experience working with vulnerable populations in their homes.
  • Preferred: BA or BS degree in social work, case management, sociology, psychology, LPN, or a related field.
  • 1-2 years of experience in social services and healthcare systems, including familiarity with state and federal programs (e.g., Medicare, Medicaid).
  • Experience with electronic records is preferred.

Work Environment:

This role involves visiting participants in their homes, which may require navigating stairs. The position entails significant driving, often exceeding 60 miles each way, and may involve assisting participants in neighboring areas. Administrative tasks will be performed at home, including typing, making phone calls, and attending virtual meetings with colleagues. The role balances participant visits with home office responsibilities, necessitating a considerable amount of sitting.
