Senior Community Care Nurse Practitioner Opportunity

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Senior Community Care team at UnitedHealth Group Inc. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to our most vulnerable populations.

Key Responsibilities:
  • Conduct comprehensive assessments and develop personalized care plans to address the unique needs of our patients.
  • Manage complex medical situations, including acute episodes and chronic conditions, with a focus on prevention and early intervention.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to patients and their caregivers to promote healthy behaviors and self-management.
  • Stay up-to-date with the latest clinical guidelines and best practices to deliver evidence-based care.
Requirements:
  • Master's degree in Nursing or Doctor of Nursing Practice from an accredited institution.
  • Active and unrestricted license to practice as a Nurse Practitioner in the state where you reside.
  • Board certification in a relevant specialty, such as Family Nurse Practitioner or Adult-Gerontology Acute Care Nurse Practitioner.
  • Current DEA licensure or ability to obtain post-hire, per state regulations.
  • Ability to gain a collaborative practice agreement, if applicable in your state.
  • Access to reliable transportation and ability to navigate various dwelling conditions.
Preferred Qualifications:
  • 1+ years of clinical experience in a long-term care setting.
  • Experience in managing complex behavioral, social, and functional needs.
  • Understanding of Geriatrics and Chronic Illness.
  • Proficient computer skills, including electronic medical records.
