Job Title: LPN Wing Supervisor

We are seeking a highly skilled and compassionate LPN Wing Supervisor to join our team at Della Infotech, Inc. in Kane, PA.

Job Summary:

The LPN Wing Supervisor will be responsible for overseeing the care of residents in our long-term care facility, ensuring that their medical needs are met and their quality of life is maintained. Responsibilities:
  • Supervise and direct the care of residents, ensuring that their medical needs are met and their quality of life is maintained.
  • Administer and record medication, treatments, and monitoring as ordered, and report and document resident response.
  • Accurately transcribe physician's orders and complete monthly summaries as scheduled.
  • Review the care plan with NAs on monthly summary day and update the care plan to reflect the resident's current condition.
  • Notify the RN Supervisor immediately of a significant change in resident's condition.
  • Report and record changes in resident condition to the RN Supervisor.
  • Communicate resident transfers to immediate family and via email to department heads.
  • Complete transfer form for all transfers.
  • Update care plans with clear, concise, time-limited goals to reflect resident condition and direct team to implement approaches.
  • Assist physicians with rounds, examinations, and treatments as needed.
  • Collect laboratory specimens and arrange for delivery to the hospital with accurate and complete requisitions.
  • On resident death, supervise and assist with post-mortem procedures, assist with collection of resident's clothing and belongings, and notify all department heads of death via email.
  • Encourage resident participation in scheduled activities.
  • Report to team changes in resident condition, care plan changes, and any special assignments to be carried out.
  • Assist RN Supervisor with annual evaluations of nursing assistants.
  • Along with RN Supervisor, write commendations and counseling records as needed.
  • Accurately complete incident reports or assist team members in writing report.
  • Document weights and vital signs and follow up as needed.
  • Clean and stock treatment trays daily.
  • Prepare daily report for the wing.
  • Count narcotics with another nurse at beginning and end of shift.
  • Maintain an accurate inventory of resident belongings.
  • Maintain work area in clean, orderly condition and perform housekeeping assignments as scheduled.
  • Submit maintenance requests as needed.
  • Maintain a clean, safe, and healthy environment and report potential safety hazards.
  • Supervise and direct resident care by nursing assistants to ensure the prescribed plan of care is carried out.
  • Perform additional duties as directed by supervisor.
  • Attend care plan conference for residents on assigned wing if requested.
  • Assist in orientation of new nursing staff.
  • Adjust staffing assignments as needed to ensure good resident care.
  • Conduct team conferences as needed.
  • Assure that staff is familiar with changes in policies and procedures.
  • Update the daily census as needed.
  • Monitor NA documentation for completeness and compliance with regulations.
  • Collect data for quality assurance studies as directed.
  • Monitor nurse aide job performance for compliance with infection control practices.
  • Provide direct care for residents as needed, within restrictions.
  • Seek clarification or guidance to perform job responsibilities when in doubt.
  • Document drug disposal on resident charts in PCC.
  • Attend in-services, department, and staff meetings, committee meetings as required.
  • Assist CNA's on the hall as needed.
Requirements:
  • BLS (AHA) certification required.
  • COVID vaccine preferred but not required.
  • Must possess a current license in PA or a temporary work permit at time of interview.
  • Must have knowledge of medical asepsis, body mechanics, personal care of residents, correct use and care of equipment, observation techniques, reporting and recording, thorough knowledge of nursing theory and its application.
  • Must be willing to possess knowledge of and comply with regulations, policies, procedures, contracts, and philosophy of the institution.
  • Must maintain a courteous and professional manner toward residents, visitors, and co-workers and a dedication to meet the needs of the elderly.
  • Must have awareness of cost containment, utilize time fully, and properly care for equipment.
  • Must possess ability to accept and follow written and verbal directives.
  • Must utilize good communication and observation skills.
