**Job Summary**

We are seeking an experienced Registered Nurse (RN) to join our Home Health team at St. Luke's Health System in Boise, Idaho. As a Home Care RN, you will provide comprehensive in-home care to patients with diverse needs, promoting independence, dignity, and overall well-being.

**Key Responsibilities**

  • Develop and implement individualized care plans to meet patients' unique needs and goals.
  • Conduct thorough assessments of patients' physical and emotional status, identifying areas for improvement and developing strategies for enhanced care.
  • Maintain accurate and detailed records of patient interactions, progress, and outcomes.

**Qualifications**

  • Current RN license in the state of Idaho and Basic Life Support (BLS) certification.
