We are seeking a skilled Speech Therapist to join our team at The Overlook, a non-profit organization dedicated to providing high-quality care and services to older adults in Massachusetts.
Job Summary:The Speech Therapist will provide skilled speech therapy services in home and skilled nursing settings, consistent with ASHA professional standards and MHS/OCAH policies. The ideal candidate will have a strong background in speech pathology and experience working with patients in various settings.
Key Responsibilities:- Complete patient assessments and evaluations to determine functional level and establish a therapeutic program.
- Develop a therapy plan of care with the overall plan of care to include goals and discharge plans.
- Document, within time frames, observations, patient's response to treatment, treatment programs, degree of progress and notable changes in patient condition.
- Communicate and coordinates patient care with other members of the patient's health care team.
- Provide patient caregiver, and family teaching related to the patient's speech therapy program and its maintenance.
- Graduate of an accredited Speech Language Pathology program.
- Current license to practice as a Speech Language Pathologist in the state of Massachusetts.
- Must possess a current Driver's License and reliable transportation.
