We are seeking a skilled LPN to provide direct patient care and collaborate with healthcare professionals to achieve optimal outcomes. The ideal candidate will possess strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

**Responsibilities:

  • Deliver high-quality patient care, including administering medications, performing vital signs, and documenting patient information.\
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.\
  • Conduct ongoing assessments of patients' physical and emotional needs, making adjustments to care plans as needed.\
  • Maintain accurate and detailed records of patient interactions, treatments, and outcomes.\
  • Participate in quality improvement initiatives to enhance patient care and services.\
  • Support and mentor new staff members in their roles as LPNs.
