We are seeking a highly skilled Clinical Pharmacist II to join our team at Christus Health. As a Clinical Pharmacist II, you will play a critical role in providing exceptional patient care and contributing to the advancement of our pharmacy department.
Key Responsibilities- Prepare and dispense drug orders per physician request according to established policies, procedures, and protocols.
- Interpret drug orders (verbal and written) and transcribe/verify into computerized patient medication record accurately.
- Compound and dispense pharmaceuticals, including non-sterile and sterile products, chemotherapy, and parenteral nutrition products accurately.
- Issue controlled substances to patient care areas and maintain records as required by law and institutional policies.
- Provide services efficiently and in a timely fashion.
- Supervise and direct pharmacy support personnel, verifying the daily activities of pharmacy technicians.
- Maintain competency required for current job title/position.
- Maintain current pharmacist license, ensuring applicable CE records and licensure are maintained in department files.
- Attend staff meetings.
- Complete all competency/skills assessment requirements.
- Provide analysis of medication use processes and contribute at least one related project per year.
- Participate in antibiotic stewardship, opioid stewardship, and shortage management activities as assigned.
- Attend interdisciplinary rounds when assigned.
- Provide services efficiently and in a timely fashion.
- Contribute to the advancement of the pharmacy department and medication management of the organization through precepting of pharmacy residents and students, assessment of new and ongoing pharmacist competencies, in-service of pharmacy and/or nursing staff, and participation in departmental or sub-committees as assigned.
- Complete one clinical project per year.
- Doctor of Pharmacy or BS Pharmacy degree required.
- One of the following required: BPS Board Certification, Residency, Fellowship, or at least 5 years of hospital experience.
- PGY-1 and PGY-2 residency completion, Board certification, or 5 years hospital experience required at hire.
- Pharmacy registration in the State of practice.
- Must have pharmacy State licensure within 60 days of start date.
- Pharmacist preceptor within 90 days.
- BLS or ACLS and PALS (pediatric practice) within 180 days of start date.
- Must accrue ASHP Residency Preceptor status within 180 days of hire (for Ministries with Residency programs).
