Job Summary

We are seeking a skilled Master Social Worker to join our team at LIFE NWPA. As a key member of our interdisciplinary team, you will provide direct social work case management services to our participants, including screening, case management, counseling, and referral.

Responsibilities
  • Participate in initial assessments, care planning, and ongoing re-evaluations of participant cases.
  • Maintain current written case management records, including periodic reassessments of program participants.
  • Provide individual and family counseling, develop and lead group counseling and activities.
  • Refer participants to community agencies or facilities, act as a liaison with such organizations, and advocate for participants.
  • Create effective relationships among staff to build and maintain a cohesive team.
  • Consult with and advise staff members on social, emotional, and cultural factors to health and medical care and the availability of social services in the community.
  • Participate in program and policy development of the social work component of the program.
  • Communicate participant changes to team members.
  • Participate in ongoing Quality Initiative training as directed.
Requirements
  • Master's Degree from an accredited School of Social Work (MSW) with one year of experience working with the frail/elderly population.
  • One year of experience working with the frail and elderly population or willingness to participate in extensive training as needed.
  • Valid state driver's license and reliable transportation.
  • Successful completion of background check, physical, drug screening, TB screening.
