Key Responsibilities:
  • Provide direct Care Management Services to a defined patient population while supervising Case Management & Social Work staff.
  • Support department Managers, Directors, and Vice President in the direction and education of all phases of the Care Management process.
  • Collaborate in a collegial manner with patients, physicians, staff, consultants, and ancillary departments.
  • Responsible for direct oversight and supervision of department-based teams; coordinate special departmental projects like patient and staff satisfaction, length of stay reduction, and service delivery improvement.
  • Maintain assignments and support departmental activities for Care Management team members.
  • Identify and resolve team issues, and work with the patient, family, and healthcare team to formulate a discharge plan for appropriate post-acute care services.
  • Gather and assess information regarding patient's needs, including physical, mental, family support, financial resources, and available community resources.
  • Develop a comprehensive care management plan addressing identified patient needs.
  • Provide oversight of PTO, disability time, and FMLA to ensure optimal staffing for service needs.
  • Directly supervise department staff, including orientation, mentoring, coaching, and evaluation.
  • Evaluate staff development opportunities and competencies, managing and evaluating Care Management staff.
  • Coordinate and initiate training/orientation programs for staff and ongoing educational presentations.
  • Maintain employee files in cooperation with the department Manager/Director.
  • Communicate and interact with internal and external customers, including Information Services, Financial Services, Ancillary departments, Third Party Payers, community social service agencies, healthcare facilities, and nursing department liaisons.
  • Assist in coordinating and scheduling meetings to support staff needs and improve department service delivery.
  • Participate in preparation, review, and monitoring of departmental budgets in coordination with departmental Manager and Director.
  • Engage in the development and maintenance of departmental policies and procedures.
  • Contribute to Performance Improvement activities related to departmental functions, system-wide initiatives, and regulatory guidelines.
  • Prioritize incoming requests and connect with Managers, Directors, or Vice President as needed.
  • Perform other related duties as requested.
