- As a Home Health LPN, you will work closely with patients, physicians, and other healthcare professionals to develop and implement personalized care plans that address their unique needs and goals.
- You will provide skilled nursing interventions, including wound care, medication management, and education on healthy living practices.
- With a focus on whole-person health, you will also address the emotional and social well-being of your patients, empowering them to take control of their health and live independently.
Some of the key responsibilities of this role include:
- Conducting comprehensive assessments of patients' physical, emotional, and social needs
- Developing and implementing individualized care plans that prioritize patient-centered care
- Providing education and support to patients and their families on managing chronic conditions, medication adherence, and healthy lifestyle choices
- Maintaining accurate and timely documentation of patient progress and care activities
To be successful in this role, you will need:
- A graduate degree from an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing
- A current nursing license in the practicing state
- At least two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care

- An estimated salary range of $ $65,000 - $85,000 per year, depending on experience
