As an Admissions Registered Nurse at BAYADA Home Health Care, you'll play a vital role in setting the stage for successful patient care, safe transitions, and positive outcomes for patients leaving Inspira Hospitals.
This specialized role is dedicated to supporting patients specifically discharging from Inspira Hospitals, ensuring they receive the highest quality care in the comfort of their own homes.
By collaborating closely with transitional care managers, hospital discharge planners, and other healthcare professionals, you'll build strong relationships that will help our patients safely meet their medical needs without entering a facility.

- Build strong relationships with transitional care teams, discharge planners, and population health
What You'll Bring to the Table:
- A current NJ RN license and at least one year of recent clinical experience
- Prior home care experience or comprehensive medical assessment experience is a plus, but not required
- Ability to work independently, manage time effectively, and communicate well with patients, families, and healthcare partners
