Urrly is seeking a skilled Registered Nurse (RN) to join our team in providing high-quality care to patients in their own homes.
Key Responsibilities:- Conduct thorough patient assessments to evaluate physical, mental, and social factors affecting patient health.
- Develop and implement individualized care plans in collaboration with patients, caregivers, and the primary Medical Practitioner.
- Provide direct care services, including wound care, infusion therapy, medication administration, and health education.
- Coordinate care by supervising LPNs and HHAs to ensure consistent, quality care for patients.
- Document and communicate patient progress to the medical team.
- Active NYS RN License and a valid NYS Driver's License.
- Minimum of 1 year recent experience in geriatric care.
- Home care experience is a plus.
- Hourly pay: $37 - $50.
- Full benefits, including health, dental, vision, 401(k), and more.
- Monday to Friday, 8am-4pm schedule.
- Travel reimbursement and tuition assistance available.
