Job Summary:
Elara Caring is seeking a highly skilled Psych Registered Nurse to join our Home Health team. As a Psych Registered Nurse, you will play a vital role in delivering high-quality care to patients in their preferred home setting.
Key Responsibilities:
- Assess and evaluate patient needs, developing individualized plans of care to address physical, psychological, social, and spiritual factors.
- Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
- Conduct regular home visits to assess patient progress, provide education, and make adjustments to care plans as needed.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
- Current, unrestricted state RN license.
- 1+ years of clinical experience in an inpatient psychiatric unit (BSN) or 2+ years (ASN).
- OR: an MSN with a concentration in psychiatric/behavioral health nursing.
- Able and willing to travel within branch/office coverage area.
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
- Associates Degree or Bachelor's Degree in Nursing is preferred.
- Previous Home Health or Hospice Experience is preferred.
- Previous experience with HomeCare HomeBase is preferred.
