We are seeking a skilled Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Physical Therapist, you will provide high-quality care to patients in their homes, helping them to recover from illness or injury and regain their independence.

About the Role:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
Requirements:
  • Current licensure as a Physical Therapist in the state of employment.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Home care experience is desired, but not required.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
  • Knowledge of therapy best practices and current evidence-based guidelines.
