Care Management Expertise
We are seeking an experienced Care Manager II to provide expert-level care coordination and guidance to our patients and their families. In this role, you will work collaboratively with interdisciplinary teams to develop and implement individualized care plans that promote optimal patient outcomes.

Duties and Responsibilities
• Utilize case finding criteria to identify patient needs and develop comprehensive care plans
• Facilitate timely documentation review with CDI specialists to ensure accurate clinical documentation
• Collaborate with physicians and other healthcare providers to ensure coordinated care delivery
• Communicate effectively with patients and families to ensure informed decision-making and seamless transitions

Qualifications
Current Tennessee RN license; Bachelor's degree in Nursing or related field; 4 years of experience as a Registered Nurse; minimum of 3 years in area of responsibility.
