Salary: $45-$60 per hour, based on experience

Job Description: As a Hospice LPN Care Specialist with Queen City Hospice, you'll play a vital role in providing high-quality patient care, working closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient. Your expertise will be valued as you assess patient conditions, provide medication management, and offer emotional support to patients and their families during this critical time.

Required Skills and Qualifications: To succeed in this role, you'll need to be a licensed LPN/LVN in the state of Ohio, possess current CPR certification, and have at least three years of nursing experience, preferably in community health, hospice, or medical-surgical settings. Strong communication and organizational skills are also essential for success in this position.

What You'll Do: Key responsibilities of this role include:

  • Providing direct patient care as defined in the State Nurse Practice Act
  • Implementing current nursing practice following comprehensive assessments and care plans
  • Documenting patient services accurately and in a timely manner
  • Assessing patient and family/caregiver education and information needs
  • Participating in the coordination of hospice services and reporting identified needs to the interdisciplinary group
