Job Opportunity

We are seeking a skilled Licensed Practical Nurse LPN to join our Assertive Community Treatment ACT team at our Wilmington, Delaware location.

This is a full-time position with a 40-hour workweek, Monday through Friday, with some flexibility to rotate and cover weekends in a office environment and community-based work setting. A company vehicle will be provided for community-based work, and business casual attire is expected.

The Licensed Practical Nurse LPN will provide coordination of behavioral and physical health care needs of program participants, monitor medication, and provide psychiatric nursing services consistent with physician's orders. Responsibilities include:

  • Collaborating with participants and assigned staff to develop, implement, evaluate, and revise recovery plans.
  • Maintaining a caseload of assigned participants and completing assigned paperwork in a timely manner.
  • Serving as a liaison to consult with community agencies and families to maintain coordination of care in the treatment process.
  • Providing ongoing assessment of participant's mental health symptoms and responding to recovery plans.
  • Ensuring annual review of participants' physical condition and assisting access to and encouraging compliance with treatment for identified physical problems.

Requirements:

  • Board Certified Licensed Practical Nurse LPN in the state of Delaware or participating in the Nursing Licensure Compact.
  • Knowledge of mental health systems and supports.
  • Knowledge of multiple support systems for substance abusers.
  • Excellent communication skills, both oral and written.
  • Ability to work independently and as part of a focused team.
  • Possession of a valid driver's license with an acceptable driving record.
  • Strong organizational and analytical skills.

Work Environment:

  • Frequent talking and listening.
  • Requires the ability to lift, carry, hold, kneel, stack, stoop, reach, and bend, which may include direct intervention with participants.
  • Local travel required, including transporting participants.
  • May be required to report to work during emergencies, including inclement weather.
  • May require crisis intervention in emergency situations.
  • Scheduling flexibility and possible on-call work are necessary.
