Family Nurse Practitioner

Molina Healthcare is seeking a highly skilled Family Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide high-quality primary care services to patients in their homes, communities, and nursing facilities.
Key Responsibilities- Conduct comprehensive medical assessments, order diagnostic tests, and develop treatment plans for patients with various health conditions.
- Provide general medical care and care coordination to adults, women's health, pediatric, and geriatric patients.
- Perform face-to-face in-person visits in various settings, including home, skilled nursing facilities, and public locations.
- Collaborate with fellow nurse practitioners to develop best practices and improve patient outcomes.
- Participate in community-based Pop Up Clinics to build relationships with the community and address gaps in healthcare.
- Drive up to 120 miles a day to various locations within the assigned region.
- Master's degree in family health from an accredited nursing program.
- Advanced computer skills, including proficiency with Word, Excel, and Electronic Medical Record.
- Current state-issued license to practice as a Family Nurse Practitioner.
- Current Basic Life Support for Healthcare Professional certification.
- Unrestricted national certification from a recognized organization, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
- 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner, ideally in a home health, community health, or public health setting.
- Previous experience in home health as a licensed clinician, especially in management of chronic conditions.
- Fluency in a language in addition to English is a plus.
