Maxim Healthcare Services is seeking a skilled LPN to provide high-quality care to pediatric and adult patients in their own homes in the Zanesville, OH area.

Responsibilities:
Utilize the nursing process to plan, implement, and evaluate patient care and services provided to clients receiving contracted services, as required

Initiate the plan of care when directed and request revisions from the prescriber as patient status and needs change

Incorporate therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals

Intervene in response to changes in patient condition and report changes to the appropriate physician or supervisor

Administer medications and treatments as prescribed and monitor their effectiveness

Review the plan of care and program protocols to identify areas for healthcare teaching, care coordination, goal setting, and discharge planning needs, as applicable

Requirements:
Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) License for the state in which the nurse practices

Current Health Certificate, as per facility Registered Nurse (RN) or Licensed Practical Nurse (LPN) contract or state regulation

Current PPD or Chest X-Ray

Current BLS card

One year prior Registered Nurse (RN) or Licensed Practical Nurse (LPN) experience preferred
