We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Firelands Health. As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional outcomes.
Key Responsibilities- Perform home care visits, collecting ongoing data and reporting to the RN Clinical Supervisor, implementing the Plan of Care.
- Provide skilled nursing care in patients' homes as ordered by the attending physician, in conjunction with the Plan of Care.
- Autonomy within your position, utilizing your training, critical thinking, and knowledge of patients to make decisions.
- Submit clear and appropriate documentation in a timely manner and document patient education.
- Communicate effectively to physicians, supervisors, and co-workers in a timely manner.
- Promote continuity of care with appropriate admissions, transfers, and charges.
- Graduate of an accredited school of professional nursing, BSN preferred, currently licensed as an RN through the Ohio State Board of Nursing.
- Prior nursing experience in Hospice or Home Health setting preferred. At least one year's medical/surgical nursing experience preferred.
- BLS/CPR Certification required.
- Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy.
- Independent decision-making skills and abilities to render nursing care in an independent setting.
