Key Responsibilities- Conduct comprehensive assessments of newly enrolled patients and provide ongoing care to ensure optimal health outcomes.
- Develop and implement personalized care plans, collaborating with patients, their legal representatives, and other healthcare professionals as needed.
- Prescribe diagnostics and interventions to prevent unnecessary acute admissions and promote cost-effective care.
- Provide education and support to patients and their families on disease management and wellness strategies.
- Participate in ongoing meetings and quality initiatives to ensure the highest standards of care and promote clinical excellence.
- Stay up-to-date on relevant medical and nursing research, technology, and best practices through ongoing education and professional development.
- Completion of a Nurse Practitioner or Physician Assistant accredited program.
- Current state RN/NP or PA license.
- Certification through a recognized professional organization (e.g., AANP, ANCC, NCCPA).
- Access to reliable transportation.
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of long-term care, community resources, and cost-effective alternatives.
- Knowledge of CPT, ICD-10, and HCPC codes.
- Knowledge of clinical standards of care and NCQA requirements.
