Job Summary:

The successful candidate will provide annual in-home assessments for members of Medicare, Medicaid, and Affordable Care Act plans. You will identify care gaps, health risks, and social determinants of health to help PCPs and health plans promote better outcomes.

Key Responsibilities:
  • Perform high-quality assessments to meet patient needs and health plan expectations.
  • Document and submit chart notes within 24 hours of the visit.
  • Address charting queries within 48 hours of receipt.
  • Attend periodic training sessions and meetings pertinent to the position.
Requirements:
  • Master's or Doctorate degree from an accredited NP or PA program.
  • Current RN and NP or PA licensure in the present state of practice.
  • Board certifications accepted: FNP, ANP, AGNP, ACNP, AGACNP, and AGPCNP.
  • Current BLS certification.
  • Valid unencumbered state driver's license.
  • Valid proof of sufficient car insurance coverage for the state appointments are scheduled to be completed in.
