Job Summary

Solvista Health is seeking a highly skilled Registered Nurse to join our team as a Clinical Care Coordinator. This is a dynamic role that requires a strong clinical background, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Coordinate and provide professional nursing care to patients, including assessment, treatment, care planning, and evaluation.
  • Serve as the primary liaison between patients, staff, and other members of the patient's care team.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with multidisciplinary teams to improve patient care processes and outcomes.
  • Provide education and support to patients and families to promote health and wellness.
  • Document patient assessment, goals, interventions, and progress in the electronic health record.
  • Participate in quality improvement initiatives to ensure high-quality patient care.
Requirements
  • Current state registered nurse license.
  • Minimum of three years professional nursing experience, preferably in an outpatient clinic setting.
  • Knowledge of professional nursing theory, healthcare laws, regulations, and practices.
  • Ability to analyze options and counsel patients and families about choices.
  • Ability to engage clients and families in the development and implementation of a care plan.
  • Ability to communicate and work effectively with diverse individuals at all levels.
Working Conditions

This role operates in a fast-paced, professional office environment and requires the use of standard office equipment. The position may require travel to conferences, meetings, and branch locations on a regular or intermittent basis. Work may involve moderate exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises.

Physical Demands

This is a largely sedentary role but can involve standing or sitting for extended periods of time, bending at the waist, and using hands and fingers to handle and file papers or operate assigned equipment. While performing the duties of this job, the employee is regularly required to talk or hear. Specific vision abilities required by this job include close vision, distance vision, color vision, and ability to adjust focus.

Conditions of Employment
  • Annual TB, federally required drug screening, and Influenza vaccination or compliance with policy and procedure.
  • Required possession of a valid state driver's license.
  • Successful candidate must submit to and pass, post-offer drug screen.
