About the Role:

As a Private Duty Nurse LPN, you will play a vital role in providing one-on-one care to our patients in the comfort of their own homes. Your expertise and compassion will make a significant difference in the lives of our patients and their families.

Responsibilities:

  • Implement physician-prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in our system of record
  • Teach and train family members on health promotion and disease management
  • Communicate effectively with physicians, clients, and staff

Qualifications:

  • Active, unencumbered LPN license in the state of practice
  • CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within the last 12 months)
  • One year of prior hands-on nursing experience
  • Reliable transportation
