Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality care to patients in their homes. As a Private Duty LPN, you will have the opportunity to build meaningful relationships with patients and their families, while also enjoying a flexible schedule and opportunities for career growth.

Key Responsibilities
  • Provide direct nursing care to patients in their homes, as prescribed by physicians
  • Record clinical and progress notes, and coordinate other patient care activities to ensure the highest quality of patient care
  • Work collaboratively with the healthcare team to develop and implement individualized care plans
  • Ensure confidentiality of patient health information and maintain accurate records
Requirements
  • Current LPN licensure in the state of practice
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
  • Current CPR certification
  • Previous home care experience preferred
