Alternate Solutions Health Network LLC is seeking a skilled Occupational Therapist to join our team in Oakton, Virginia.

We are looking for a compassionate and detail-oriented Occupational Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Occupational Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.

• Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
• Evaluate patient needs and develop individualized plans of care to promote independence and functional ability.
• Implement and modify treatment plans as needed to ensure optimal patient outcomes.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
• Document patient progress and outcomes in accordance with agency policies and regulatory requirements.

• Current license to practice as an Occupational Therapist in the state of Virginia.
• Minimum one year of experience as an Occupational Therapist in an acute care setting, with preference given to those with home care experience.
• Valid driver's license and auto insurance.
• Ability to lift, bend, and move patients as needed.
