About the Role

We are seeking a skilled Community Care RN to join our team at Providence PACE, a Program of All-Inclusive Care for the Elderly. As a Community Care RN, you will provide and supervise professional, independent nursing care for our patients, promoting their optimal health and independence.

Key Responsibilities
  • Assess, plan, and implement individualized care plans for patients, ensuring their needs are met and their health is optimized.
  • Supervise and delegate tasks to other healthcare professionals, ensuring high-quality care is delivered.
  • Collaborate with the interdisciplinary team to develop and implement care plans that address the physical, emotional, and social needs of patients.
  • Provide education and support to patients and their families, empowering them to take an active role in their care.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in care delivery.
Requirements
  • Current Oregon Registered Nurse License
  • National Provider BLS - American Heart Association certification
  • Relevant certifications applicable to the position, as specified by the Nurse Manager
  • Coursework/Training relevant to the position, as specified by the Nurse Manager
Preferred Qualifications
  • Bachelor's Degree in Nursing
  • 1 year of Home health RN experience
