Job Title: Social Worker

Job Summary:

Genesis HealthCare, Inc. is seeking a dedicated and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in promoting the well-being and quality of life for our patients and residents.

Responsibilities:

* Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the social and emotional needs of our patients and residents.

* Conduct thorough assessments of patients' and residents' social and emotional needs, and develop plans to address these needs.

* Provide education and support to patients, residents, and their families on topics such as advance care planning, end-of-life care, and community resources.

* Develop and maintain relationships with community-based agencies and services to ensure seamless transitions and referrals.

* Participate in quality improvement initiatives to enhance the overall quality of care and services provided.

* Maintain accurate and timely documentation of patient and resident interactions, assessments, and care plans.

Requirements:

* Bachelor's degree in social work or a related field.

* Possession of any required certifications or licenses to practice in long-term care.

* Excellent communication and interpersonal skills.

* Ability to work effectively in a fast-paced environment.

* Strong problem-solving and critical thinking skills.
