Our team carries low patient caseloads and never has on-call responsibilities. We empower our staff with manageable workloads, support, and flexibility, ensuring rewarding careers in hospice care.

As a member of our interdisciplinary team, you will:

  • Provide direct patient care as defined by the State Nurse Practice Act
  • Implement current nursing practice based on comprehensive assessments and plans of care
  • Document patient services accurately and timely, reflecting the plan of care
  • Assess and educate patients and families/caregivers on relevant diagnoses and self-plans of care
  • Participate in coordinating hospice services, reporting identified needs to the interdisciplinary group
  • Manage pain and symptoms effectively, evaluating patient responses to treatments/medications

To excel in this role, you should possess:

  • A graduate degree from an accredited practical nurse or vocational nursing program
  • Three years of nursing experience, preferably in community health, hospice, or medical/surgical settings
  • Current licensure as an LPN/LVN in the practicing state
  • CPR certification
  • Demonstrated excellent verbal and written communication, and organizational skills
  • A valid driver's license with insured vehicle
  • An understanding of the hospice concept and the needs of the terminally ill
