Centinela Hospital Medical Center is seeking a skilled Licensed Vocational Nurse to join our Case Management team. As a key member of our team, you will be responsible for coordinating the activities of the Case Management Department and developing and implementing discharge plans for patients.
Responsibilities- Coordinate the activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker.
- Develop and implement discharge plans for patients, ensuring a safe and effective transition to post-acute care.
- Maintain and coordinate Medicaid state submission forms for presentation to on-site reviewers.
- Assist with case management referrals, discharge planning, and other assigned tasks.
- Provide clear verbal and written communication of discharge planning information to stakeholders.
- Active state LVN license required upon hire and maintain current.
- Current BCLS (AHA) or (ARC) certificate required upon hire and maintain current.
- Experience in basic to intermediate computer skills/knowledge.
Centinela Hospital Medical Center offers a competitive compensation package, including a 401K retirement plan, medical, dental, and vision coverage, tuition reimbursement, and other benefits. A reasonable compensation estimate for this role is $28.00 to $37.68, which includes estimated wages, benefits, and other forms of compensation.
