Care Manager RN Job Summary

We are seeking a skilled and compassionate RN to join our Care Management team at Care Around the Block. As a Care Manager RN, you will play a vital role in providing quality professional care management services to our clients and their families.

Key Responsibilities:
  • Conduct thorough care plan assessments to identify clients' needs and develop comprehensive plans of care.
  • Advocate for clients and their families, ensuring their voices are heard and their needs are met.
  • Collaborate with healthcare professionals, community resources, and other stakeholders to coordinate care and services.
  • Monitor clients' health status and well-being, providing ongoing support and education as needed.
  • Document all interactions and care plans accurately and efficiently, adhering to company standards and regulations.
Requirements:
  • TN Licensed Registered Nurse (Associates or Higher required, BSN preferred)
  • CCM Certification preferred
  • Minimum of 3 years of experience in local human service or medical field, with knowledge of community resources and services
  • Reliable transportation and vehicle insurance
  • Computer proficiency in Microsoft Word, Excel, and Outlook, with ability to learn new software
  • Ability to pass a Criminal Background Check, Drug Screen, and DMV Check at hire and randomly as required by company policy
