Job Summary

As a Home Health Nurse with Della Infotech, you will play a vital role in providing high-quality patient care in the comfort of their own homes. We are seeking a skilled and compassionate Registered Nurse to join our team and make a meaningful difference in the lives of our patients.

Key Responsibilities

1. Conduct thorough assessments of patients to determine their home care needs and develop personalized care plans.

2. Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.

3. Educate patients and their families on proper self-care techniques and provide guidance on managing chronic conditions.

4. Collaborate with healthcare teams to ensure seamless care coordination and communication.

5. Maintain accurate and detailed records of patient interactions and care provided.

Requirements

1. 1 year of experience in Adult/Pediatric Home Health care.

2. Graduate of an accredited school of nursing with 1 year of recent acute care experience.

3. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

4. Good typing skills.

5. Wound care, wound vac, catheter, acute care experience.

Preferred Qualifications

1. Current CPR certification.

2. California state license.
