About the Role

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing exceptional care to adult and geriatric patients in their homes.

This is a fantastic opportunity to work with a multidisciplinary healthcare team, utilizing your expertise to develop and implement individualized care plans that promote independence and improve quality of life.

Responsibilities
  • Conduct home visits to assess clients' physical needs and develop personalized care plans
  • Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance
  • Implement manual therapeutic exercises, gait training, and other interventions to improve clients' physical function
  • Collaborate with the nursing team to revise and update care plans as needed
  • Educate clients and their families on rehabilitative care and activities to promote independence
Requirements
  • Current license as a Physical Therapist in Massachusetts
  • Minimum of one year of recent clinical experience as a licensed Physical Therapist
  • Ability to travel to patient homes as assigned
